After School Program Enrollment

Step 1: Fill Registration form (Please click to download). Optionally fill out ACH authorization form to setup AutoPay.

Step 2: Please Print out and Return signed and completed forms, along with $200 deposit to secure a spot (plus $80 registration fee).

Please make check payable to “Yu Ying Learning Center” and mail or deliver the form and deposit check to our address:

Step 3: You will receive an enrollment confirmation by email after we have your registration package.

  • Registration is on a first come, first serve basis.
  • Deposit will be applied to first installment of tuition and fees.
  • Age requirement: All candidate must be 4 years and 9 months on the first day he/she attends our after school.
